Booking and Personal Service Terms and Conditions

By booking or participating in a tattoo or any related service at Needle Therapy Tattoo, you hereby agree to the following Terms and Conditions.

By booking an appointment, you acknowledge that you have read, understand, and agree to be bound by these Terms. If you make a booking on behalf of other participants, you guarantee that you have the authority to accept and do accept these Terms on behalf of the other participants in your party.

1. THE BOOKING CONTRACT

Your booking is confirmed, and a contract exists when your artist or authorized agent issues a written confirmation after receipt of the applicable (non-refundable) deposit amount.

Please check your confirmation carefully and report any incorrect or incomplete information to your artist or authorized agent immediately.

You must be at least 18 years of age to book a tattoo appointment. Failure to meet the age requirement at the time of your appointment will result in the cancellation of your appointment and forfeiture of your deposit.

You agree to provide full, complete, and accurate information to Needle Therapy Tattoo.

2. BOOKING ON BEHALF OF OTHERS

By booking on behalf of others (e.g., a group booking), you are deemed the designated contact person for every participant included in that booking. This means you are responsible for making all payments due, notifying the artist or agent of any changes or cancellations, and keeping your party informed.

By booking on behalf of others, you represent and warrant that you have obtained all required consents and verified the age of those involved. You are responsible for ensuring that any information provided is accurate and complete. Needle Therapy Tattoo will not be held liable for any errors or omissions in the information provided.

3. REQUIRED MEDICAL INFORMATION

You must provide any medical information reasonably requested by Needle Therapy Tattoo and may be required to complete a waiver form, including medical details relevant to the procedure.

Medical forms are mandatory for any procedure involving breaking the skin barrier.

By booking, you agree to complete the medical form honestly and disclose all relevant medical information. Needle Therapy Tattoo will handle your information in accordance with its privacy policy.

Needle Therapy Tattoo reserves the right to request further information or professional medical opinions if necessary. Services may be denied if your physical or mental condition is deemed unsafe for the procedure.

Pregnant or breastfeeding individuals are not eligible for tattoo services. Failure to disclose pregnancy or breastfeeding status will result in appointment cancellation and forfeiture of the deposit.

4. SPECIAL REQUIREMENTS

Any special requirements must be disclosed at the time of booking. Needle Therapy Tattoo will make reasonable efforts to accommodate requests, but this may not always be possible.

Certain procedures may be inaccessible due to mobility or physical limitations. If mobility accommodations are needed, please notify us in advance. Failure to disclose special requirements may result in the inability to proceed with your appointment, and Needle Therapy Tattoo will not be held liable for associated fees or injuries.

5. AGE REQUIREMENTS

• Minimum Age for Tattoos: 18 years old.

Minors must always be accompanied by an adult. Tattoo services are not available to minors under any circumstances.

6. DEPOSITS

• All deposits are 100% non-refundable.

• Deposits are required to secure your booking and are specific to the agreed-upon service.

Rescheduling Policy:

• Appointments can be rescheduled up to two times with at least 72 hours’ notice.

• Rescheduling with less than 72 hours’ notice or a third reschedule will result in forfeiture of the deposit.

Cancellation Policy:

• Deposits are forfeited if the appointment is canceled with less than 72 hours’ notice.

• Changes to the agreed artwork or placement may require a new deposit.

7. DETAILS REQUIRED FOR BOOKING

You must provide reference photos or other necessary details as requested by your artist. Failure to provide these details may result in the cancellation of your booking and forfeiture of your deposit.

Needle Therapy Tattoo will not be held responsible for delays, errors, or cancellations caused by incomplete or inaccurate information provided by the client.

8. CANCELLATION BY THE CLIENT

You may cancel your appointment by notifying your artist. Cancellation fees or forfeiture of deposits will apply based on the timing of your cancellation:

• More than 72 hours’ notice: Deposit will be held for 90 days for rescheduling. After 90 days, the deposit is forfeited.

• Less than 72 hours’ notice: Deposit is forfeited.

9. GUARANTEE OF SERVICE AND CANCELLATION BY ARTIST

If your appointment is cancelled by your artist:

• More than 72 hours’ notice: You may choose to reschedule or receive a full refund of your deposit.

• Less than 72 hours’ notice: You may choose to reschedule with a discount equivalent to your deposit or receive a full refund.

• Force Majeure (e.g., emergencies): You will be offered a rescheduled appointment.

Needle Therapy Tattoo is not responsible for incidental expenses, such as travel or lodging costs, due to cancellations.

10. LIABILITY

Needle Therapy Tattoo, its owner, employees, and agents shall not be held liable for any damage, loss, injury, or death resulting from services provided.

11. IMAGES AND MARKETING

By participating in tattoo services, you agree that images or videos taken during your session may be used for marketing purposes by Needle Therapy Tattoo. You grant a royalty-free, perpetual license for these images to be used without additional compensation.

12. PRIVACY POLICY

By providing personal information, you agree to the terms of Needle Therapy Tattoo's privacy policy.

13. AMENDMENTS

Needle Therapy Tattoo reserves the right to update or amend these Terms at any time. Changes will take effect 10 days after being posted online.
